Calidad de la tomografía contrastada de abdomen para la estadificación del cáncer gástrico en pacientes candidatos a manejo quirúrgico, en una institución de alta complejidad de la ciudad de Medellín, Colombia

Objetivos: En la actualidad, el cáncer gástrico ocupa el quinto lugar en prevalencia y es la tercera causa de muerte por cáncer en el mundo; la estadificación del cáncer gástrico, por medio de la tomografía axial computarizada (TAC) contrastada de abdomen, se ha convertido en el eje de la toma de decisiones para el manejo adecuado de este tipo de cáncer. Esta investigación busca evaluar la calidad de la TAC contrastada de abdomen para la estadificación del cáncer gástrico, en un hospital de alta complejidad de la ciudad de Medellín, Colombia.
Métodos: Se hizo la evaluación retrospectiva de 160 pacientes con diagnóstico de cáncer gástrico confirmado por histología, en una institución de tercer nivel ubicada en Medellín, Colombia, entre enero de 2015 y septiembre de 2022. En 50 pacientes que cumplieron los criterios de inclusión y exclusión se evaluaron y se correlacionaron los resultados de la TAC contrastada de abdomen preoperatoria y los hallazgos de la anatomía patológica en la pieza quirúrgica de la gastrectomía.
Resultados: Se encontró una precisión para el estadio tumoral (T) del 30 % (15/50) y una precisión para el estadio ganglionar (N) del 32 % (16/50). La precisión diagnóstica de la TAC contrastada de abdomen para el carcinoma gástrico temprano fue del 75 % con una alta especificidad (91 %) y una moderada sensibilidad (83 %).
Conclusión: La TAC contrastada de abdomen en la población estudiada tuvo un papel limitado en la estadificación del cáncer gástrico, tanto para el estadio tumoral (T) como para el estadio ganglionar (N).
