Revisión sistemática de la implementación de la técnica de radioterapia Lattice en el manejo del cáncer

Objetivo: Realizar una revisión sistemática de la literatura sobre el tratamiento de radioterapia mediante técnica Lattice para identificar, describir y organizar la evidencia disponible sobre estudios que evalúen su eficacia y seguridad.
Métodos: Se diseñó una búsqueda en PubMed/Medline de estudios de tipo experimental y observacionales retrospectivos y prospectivos, que incluyeran pacientes pediátricos y adultos con patología oncológica tratados con técnica Lattice o GRID-3D, con el fin de evaluar desenlaces clínicos de eficacia y de seguridad. Para el proceso de selección de los estudios se siguieron las directrices PRISMA. Los resultados se resumieron en forma narrativa y en tablas.
Resultados: Nueve de los once estudios incluidos fueron observacionales. La mayoría de los pacientes incluidos en estos estudios tuvieron neoplasias localizadas en cabeza y cuello y la histología predominante fueron los sarcomas. Se identificó un estudio experimental no comparativo que evaluó la técnica Lattice en pacientes con cáncer de próstata, mientras que otro estudio cuasi experimental incluyó pacientes con cáncer de pulmón. Fueron diversos los esquemas, las dosis y las técnicas propuestos.
Conclusión: Lattice es una técnica prometedora en términos de control local y de control de los síntomas, según los reportes provenientes de estudios pequeños, en su mayoría observacionales. Sin embargo, hacen falta estudios experimentales que comparen la técnica con los estándares actuales de manejo para obtener evidencia con rigor metodológico en desenlaces de eficacia y seguridad para la implementación de la técnica en la práctica habitual.
