Only 12% of patients diagnosed with colorectal cancer are under 50 years of age, and 37% of these are rectal tumors. The most common histological form of colon cancer is adenocarcinoma. However, rare histologies can be found in the colon and rectum. Squamous cell carcinoma of the colon is a rare tumor with an estimated incidence of less than 1 per 100,000 patients affected by colorectal tumors. The localized disease does not have a standard treatment approach, but a combination of surgery and chemotherapy with or without (neo)adjuvant radiotherapy is proposed. We describe the clinical case of a young adult patient with no relevant medical history, diagnosed with transverse colon squamous cell cancer who underwent surgery achieving negative oncologic margins. The patient presented with relapse in the small intestine managed with chemotherapy and surgery with a complete response to treatment.
